چکیده

Several tests of radiology have been designed to predict difficult laryngoscopy, but there were few relative studies of Chinese patients. Our study is to investigate the accuracy of lateral neck radiography in predicting difficult laryngoscopy in Chinese patients. The operative database of Xuanwu Hospital, Capital Medical University, was reviewed and all the patients who had performed lateral neck radiography before thyroid surgery from February 2014 to February 2015 were recruited. The results of Modified Mallampati test (MMT) and Thyromental distance (TMD) of the patients were obtained from the Database. Two trained reviewers independently measured α’ (the angle between hyoid cartilage, root of epiglottis and arytenoid cartilage) and several other angles and parameters on the X-ray. Receiver operating characteristic curves (ROC) were drawn and the areas under curves (AUC) were calculated to analyze the accuracy of each indicator. The cut off points were determined by maximizing the Youden index. In the study, 114 patients were included. The most accurate indicator of lateral neck radiography was α’, of which AUC was 0.896 (95% confidence interval [0.813, 0.979]). The cut off point of α’ was set as 85.52. The clinical test MMT (47% and 75%) has lower sensitivity and specificity than α’ (87% and 86%) and other indicators have lateral neck radiography. The diagnostic accuracy of lateral neck radiography in prediction of difficult laryngoscopy in Chinese patients may be superior to MMT and TMD. However, the method of lateral neck radiography is not perfectly precise.

Comparison Upper Lip Bite Test and Mallampati Test in the Prediction of Difficult Laryngoscopy

Background and Objective: Intubation is the most common method for the management of the upper airway in general anesthesia. In this regard, the prediction of the ease or difficulty of intubation is of paramount importance for the anesthesia team. The main cause of anesthesia-related mortality is failed airway management. The present study aimed to compare the accuracy of the upper lip bite (UL...

Prediction of difficult laryngoscopy in obese patients by ultrasound quantification of anterior neck soft tissue.

In 50 morbidly obese patients, we quantified the soft tissue of the neck from the skin to the anterior aspect of the trachea at the vocal cords using ultrasound. Thyromental distance, mouth opening, limited neck mobility, modified Mallampati score, abnormal upper teeth, neck circumference and sleep apnoea were assessed as predictors of difficult laryngoscopy. Of the nine (18%) cases of difficul...

بررسی ارزش تشخیصی گاز گرفتن لب فوقانی و فاصله بین دندان‌های ثنایای بیمار در پیش بینی لارنگوسکپی و لوله گذاری سخت نای

Background and purpose: Background and Aim: Difficult laryngoscopy and tracheal intubation is a major cause of morbidity and mortality during anesthesia. Its prediction is important for the anesthesiologists. Upper Lip Bite Test (ULBT) has been recently used. We aimed to evaluate ULBT, Interincisor distance (IID) and three fingers accuracy and their possible correlation to predicting difficulty...

The effect of patient positions, Sound production and neck rewind on Mallampati grading and classification to predict difficult mask ventilation in patients

Aims and background: The difficulty of ventilation with masks is one of the most common causes of severe respiratory consequences in the field of anesthesia. Objective: To evaluate the effect of patient position, voice production and neck rejection on the Mallamapati grade, in order to predict the difficulty of ventilation with mask in patients. Materials and Methods: This research was of a pr...

The diagnostic validity of clinical airway assessments for predicting difficult laryngoscopy using a grey zone approach

OBJECTIVES The diagnostic validity of clinical airway assessment tests for predicting difficult laryngoscopy in patients requiring endotracheal intubation were evaluated using receiver operating characteristic (ROC) curve analysis and a grey zone approach. METHODS In this prospective observational study, patients were evaluated during a pre-anaesthetic visit. Predictive airway assessment test...
